Health Insurance

Employee Medical, Dental, and Vision Benefits

Medical Plans

The City of Glendale offers eligible employees and dependents the choice of three comprehensive medical plans through Blue Cross Blue Shield of Arizona.

We offer an EPO (Exclusive Preferred Organization), PPO (Preferred Provider Organization), HDHP (High Deductible Health Plan) that is paired with a Health Savings Account (HSA).

Prescription Drug Program

All of our medical plans offer a retail and mail order prescription drug program. The mail order program is administered through Optum RX and offers a 90-day maintenance program, which allows you to purchase 3 months for the price of 2.

Dental Plans

The City of Glendale offers eligible employees and dependents the choice of two dental plans, both offered through Delta Dental.

Vision Plan

The City of Glendale offers eligible employees and dependents a PPO (Preferred Provider Organization) vision plan through United Healthcare Vision.

The United Healthcare Vision Plan network includes independent providers and retail chains such as Visionworks, Costco Optical, America's Best, Walmart Vision Center, Target Optical, WarbyParker.com, Eyeglass World, Nationwide Vision, and GlassesUSA.com. Benefits for glasses or contacts are available once per 12 months per enrolled member.

Flexible Spending Accounts (FSA)

Medical FSA

The City of Glendale offers eligible employees the option for a pre-tax savings account that can be used for health care expenses (e.g. co-pays, co-insurance, prescription medications, dental care, and eyeglasses).

Dependent Care FSA

The City of Glendale offers eligible employees the option for a pre-tax savings account that can be used for the care of a child (e.g. babysitting and day care), spousal, dependent or elder care.

Health Savings Accounts (HSA)

If you enroll in the High Deductible Health Plan (HDHP) you are automatically enrolled in the Health Savings Account, a pre-tax savings account that can be used for health care expenses (e.g. deductibles, co-insurance, prescription medications, dental care, and eyeglasses).

Life Insurance

Life Insurance, Supplemental Life and Commuter Life

Basic Life Insurance and Accidental Death and Dismemberment (AD&D)

All regular status employees who work at least 30 hours or more per week have basic life and accidental death and dismemberment insurance paid for by the City of Glendale. Coverage is your annual salary rounded to the nearest thousand dollars.

Spouse and/or Dependent Child Life is also provided at no cost for a spouse and/or child covered on any of our healthcare plans (medical, dental, and/or vision) on a policy amount of $1,000 each.

Optional Supplemental Life Insurance

Supplemental Life Insurance policies for the employee and dependents are available for purchase:

  • Employee Optional Life and AD&D – Coverage is available in increments of $10,000 up to $750,000.
  • Spouse Optional Life and AD&D – Coverage is available in increments of $15,000 up to $250,000.
  • Dependent Child Life - Coverage in the amount of $10,000, $15,000 or $20,000 per child.

Commuter Insurance

All employees who work at least 20 hours or more per week are covered under a Commuter life insurance policy in the amount of $200,000 paid for by the City of Glendale.

Retirement Plans

ASRS, PSPRS and Deferred Compensation

Arizona State Retirement System (ASRS)

This is a mandatory retirement program for non-sworn (civilian) employees at the City of Glendale. Eligible participants must work at least 20 hours a week for 20 weeks in a fiscal year. ASRS provides a pension plan, long-term disability insurance, retiree health insurance and survivor benefits. Contributions begin immediately upon employment. This benefit is a shared contribution between the City and the employee.

Public Safety Personnel Retirement System (PSPRS)

This is a mandatory retirement program for employees who work in sworn public safety positions who work at least 40 hours a week. PSPRS provides a pension plan, disability insurance, retiree health insurance and survivor benefits. Contributions begin immediately upon employment. This benefit is a shared contribution between the City and the employee.

Deferred Compensation - 457 Retirement Plan

During your employment with the City, you may elect to participate in a deferred compensation plan. This plan can help you defer money to help you save toward your retirement goals.

Holidays, Vacation and Sick Leave

Paid Holidays

The City of Glendale is pleased to offer its employees pay or paid time off for twelve paid holidays per year.

Please refer to Employee Policy Manual regarding when the floating holiday hours can be scheduled.

Vacation Leave Accrual

All regular status full-time and temporary employees accrue annual vacation leave on a per-hour-paid basis. Eligibility to use vacation credits shall begin after completion of one month of regular status employment.

Vacation Leave Accrual Rates:

Less than 5 years of service = 5.33 hours per pay period (138 hours per year)

  • 5 - 10 year of service = 6.12 hours per pay period
  • After 10 years of service = 7.6 hours per pay period
  • The accrual limit is 400 hours.

Sick Leave Accrual

All regular status full-time and temporary employees shall accrue sick leave benefits on a per-hour-paid basis. Eligibility to use sick leave benefits shall commence after completion of one month of regular status employment.

  • Regular employees earn .055 hours of sick leave per hour paid.
  • Temporary employees earn .034 hours of sick leave per hour paid.

Disability Insurance

Short Term and Long Term Disability

Short Term Disability

Benefit eligible employees are able to participate in the Short Term Disability Plan. This benefit is provided at no cost to the employee. Short Term Disability is available if a medical condition prevents the employee from working for at least 3 months.

Long Term Disability

Benefit eligible employees are able to participate in the Long Term Disability Plan (LTD). A portion of the cost of the LTD plan is funded by the employee's retirement contribution. Long Term Disability is available if a medical condition prevents the employee from working for at least 6 months.
