- Form W-9: Complete this form to ensure accurate tax reporting and compliance.
- Direct Deposit Authorization Form: Set up or update your direct deposit information for timely and secure rent payments.
- Rent Increase Request Form: Submit a formal request to adjust rent amounts, in accordance with program guidelines.
- New Vendor Information Form: Provide your vendor information for administrative and payment processing.

HCV Landlord Program
What is the Housing Choice Voucher Landlord Program?
Here’s how it works:
- Approval: The Glendale Housing Division will first check and approve the rental unit to make sure it meets standards and that the rent is fair. The Unit will be inspected using NSPIRE (a form has been provided for you to easily know the expectations and to be prepared).
- Lease Signing: Once approved, the tenant will sign a lease with you, the landlord.
- Rent Payment: The HCV program will help pay a part of the tenant’s rent. The tenant may also need to pay a portion of the rent directly to the landlord.
